Great analogy! I find your office visits begin with the bedside, goes to the listening, and then the chart reading. At this point the conclusion; which I observe comes after a quiet moment where you decide on the type of sentence that would have maximum impact-- a nudge, a request, an acknowledgment that the patient will not listen but what the heck he might surprise me or an admonishment that takes the patient by surprise. I have received all of those and they work for me so thank you. And Thank you Dr Clyne .

When seeing patients i usually pause and read my last office note, skim specialists’ notes, and then review their reason for being here today. As you know it’s by necessity all quite rushed and manic. It would be nice to take hours to do this properly. I create a skeleton of diagnoses to review during the visit, and then head in. On my better days, I step away from the framework and let the patient lead me to the crucial discussions. And often they tell me what the problems AND solutions are... I just need to help them create their own framework for understanding and action.

You describe a great mentor. Glad to add his mantra to my best intentions as a doc!
