Each week you find new ways to keep the stories fresh and interesting, leaving your audience in suspense till next week being the latest one. Brilliant posts, you keep them real, engaging, and stimulating. Maybe you should consider adding a publisher to your business model.

Dr. Bregman, when you shifted from a being a Doctor first, to being a business owner first, you lost me. I get that there are many challenges in pursuing your (chosen) career and also less financial rewards now for you- and other GP Doctors- than there used to be. I wish this were not the case. While investment bankers, lawyers and youtube posters are hugely rewarded for their 'talents', healthcare professionals are not richly rewarded.

First a doctor - always. Second a business owner, and then only in the service of practicing medicine in a way that would not be possible otherwise.

The idea of writing about it is to give a "behind the curtain" view of medicine from nuts to bolts - the $ side is not always discussed but always a factor wherever you practice.

Another big thank you. We've moved back to our previous home in DC after a fellowship year in NYC, and I'm here to tell you, we can not find a decent family practice anywhere near us. We live in a fairly prosperous suburb, but it's a family practice wasteland. I almost drove to NYC last week just to see you for a nasty ailment that decked me. The alternative: tough it out with fingers crossed, which I did. Not good. Miss you!
